2016 SGIM SDH interest group
There is a number of recently developed web-based platform/apps to connect patients and case managers/community health workers with community resources such as Healthify, Wellist, Purple Binder, TavHealth, Docent Health, AuntBertha. None of these (except Healthify) focus specifically on social determinants of health specifically and these tools do not help physicians assess the impacts of SDH on patients and connect them to resources as part of clinical workflow. Please feel free to check out these resources. A group of us is looking at checklist and algorithms that would lead physicians to the best community resource or service for a particular issue. We hope to share our preliminary work at the next SGIM SDH IG meeting.
